Insurance: I am a provider for the following insurance plans: Cigna, Surest, UnitedHealthcare® Choice and Choice Plus, UnitedHealthcare® Core and Core Essential, UnitedHealthcare EDGE®, UnitedHealthcare Navigate®, UnitedHealthcare® NexusACO™, UnitedHealthcare® Options PPO Plans, UnitedHealthcare® Select and Select Plus, UnitedHealthcare SignatureValue® Plans, and UnitedHealthcare® Tiered Benefit Plans.
I am also on several EAP panels, including Empathia/Life Matters, Health Advocate, and PAS.
For all other insurance, if you have a PPO insurance plan, then you may be able to see me as an out-of-network provider.* (see below) I can provide you with a monthly statement/receipt (sometimes called a superbill) to submit to your insurance provider for reimbursement. Please keep in mind that insurance companies require a diagnosis to approve your claim.
Flex Spending Accounts: Therapy is usually covered by your flex spending or health spending account. I can provide you with a monthly statement/receipt.
Providing letters or other services not covered by insurance will be charged separately. A sample fee for a letter is $100. Note: I do not evaluate for or provide letters regarding Emotional Support Animals.
